Casus primary failure of eruption
 8 jarige patiënt met primaire retentie van blijvende molaren.
 In middenlijn: mesiodens, boventallig element.
 Melkelement nog aanwezig
 Blijvende eruptie incisieven nog tegen gehouden
 Zijdelingsedelen: melkmolaren geïmpacteerd, premolaren over elkaar geprojecteerd, OK retentie melkmolaar en kipping buurelementen
3e vlak.
 Diepe beet
 Terugliggende onderkaak (retrognathie)
 Secundaire retentie: stukje doorgebroken van element (eruptie is begonnen) maar vastgroeien  bot gaat mee groeien.
 Primaire retentie: wortels breken door maar element zit nog wat vast.
o Vb. infrapositie van melkelement en kaak groeit mee.
 Tweede melkmolaren extreem gereteneerd

 Verwijderen boventallige 21
 Afwachten 36 en 46 met elastiektractie in beweging krijgen
 85 verwijderen
 Doel: eruptie van elementen die in kaak liggen

 Kipping 46
 Premolaren dicht bij elkaar
 Als de weg vrij is  normale doorbraak kan plaatsvinden
14 jaar
 Diepe beet opheven door plaat

Sinuslifting met augumentatie
Parathereoïd gen afwijking: PTH1R-gen  verticaal trekken met elastieken lukt niet.
 Wat ankylose had kunnen zijn is wellicht deze afwijking.

 FAS vroeg in ontwikkeling een deficiëntie in neurale plaat.

o Zwaar alcoholgebruik moeder
o 2/1000 bij kinderen in VS
o Schade aan zenuwweefsel, gestoorde migratie/proliferatie.
o Latere fase: constrictie bloedvaten in navelstreng  onvoldoende bloedtoevoer naar foetus.
 Symptomen
o Laag geboortegewicht, klein hoofd
o Microcefalie
o Mentale retardatie
o Slechte spiercoördinatie
o Hyperactiviteit/autisme/onaangepast gedrag/gewetenloos
 Kenmerken uiterlijk
o Kleinere hoofd omtrek
o Ogen dicht bij elkaar

o Geen duidelijke overgang bovenlip en philtrum

 Timing en dosis invloed op foetus. Daarnaast omgevingsfactoren en genetische factoren.
